According to my documentation, the configuration words have to be sent
to the interrupt controller *before* the initialization can be
started; at least, that is how it is described in the German book
"PC-Hardwarebuch" by Hans-Peter Messmer and Klaus Dembowski.

So if I get this right, you first have to send ICW1 to ICW4 to the
master and slave PICs at port 21h/a1H before you can send the
initialization command to port 20h/a0h.
